属枕先露分娩机制中的一部分。当胎儿通过衔接、下降、俯屈、内旋转，仰伸等以后，胎头已娩出。当胎头仰伸时，胎儿双肩径沿左斜径进入骨盆入口，胎头娩出后，为使胎头与胎肩恢复正常关系，胎头枕部向左旋转45°时，称为复位。胎肩在盆腔内继续下降，前(右)肩向前、向中线旋转45°时，胎儿双肩径转成与出口前后径相一致的方向，胎头枕部露在外继续向左旋转45°，以保持续胎头与胎肩的垂直关系，称为外旋转。
