内源性雌激素
内源性雌激素是一种具有广泛生物活性的甾体化合物,含有18个碳原子。其主要来源于卵巢,少量来自肾上腺皮质,妊娠期胎盘亦可产生大量雌激素。合成途径基本一致。卵巢内的卵泡膜细胞和颗粒细胞协同产生雌激素,主要为雌二醇和雌酮,前者主要由睾酮衍化,而后者由雄烯二酮转化,雌三醇为其代谢产物。雌二醇功能最强,雌酮次之,雌三醇最弱。妊娠期母体胆固醇经胎盘、胎儿肾上腺协同产生雌三醇。卵巢的雌激素水平随卵泡的发育成熟呈周期性变化。95%的雌激素在血循环中与特异的性激素结合球蛋白相结合,只有处于游离状态的雌激素具有生物学活性。雌激素不仅具有促进和维持女性生殖器官发育和第2性征的生理作用,而且对机体的代谢、内分泌、网状内皮系统、心血管系统、骨骼的生长和成熟及皮肤等方面均有明显的影响。雌激素对垂体和下丘脑产生反馈调节,主要表现为抑制FSH的分泌和促进LH的分泌。妇女体内雌激素水平从青春期开始上升,性成熟期达高峰,更年期开始呈下降趋势。
