病变以升主动脉最为显著，主动脉中层肌肉和弹性组织被*梅毒螺旋体侵入破坏产生炎症。主动脉壁逐渐变得松弛，血管内膜有皱褶出现，上面覆盖闪亮珍珠状物质的大斑块。有病变的主动脉内"树皮"样改变的出现是梅毒性主动脉炎的特征。梅毒性主动脉炎常见于后天性梅毒，在梅毒的早期即可发生梅毒性主动脉炎，但出现临床表现往往在发病后10～30年。10％的病人可发生*主动脉瘤、*主动脉瓣关闭不全、*冠状动脉口狭窄等并发症。
