人体在清醒而极端安静情况下,不受精神紧张、肌肉活动、食物和环境温度等因素影响时的能量代谢率。测定基础代谢率,要在清晨未进早餐以前,静卧休息半小时(但要保持清醒),室温维持20℃上下,按间接测热法利用仪器进行测定。基础代谢率的单位为KJ/m2/h(千焦/平方米/小时),即每小时每平方米体表所散发的热量千焦数。在同一性别、体重和年龄组的正常人中基础代谢率很接近,其中约90%以上的人其代谢率与平均值相差不超过15%。故临床上以此百分值作为正常值的界限。超过这一界限就被认为基础代谢异常。如甲状腺机能亢进的患堵塞,其基础代谢率可比正常值高20~80%;而甲状腺机能低下者则比正常值低20~40%。基础代谢率的测定是临床上诊断甲状腺疾病的简便而有效的方法。其他如肾上腺皮质和垂体前叶激素分泌不足时,也可表现为基础代谢率降低。体温升高时,基础代谢率也升高。通常体温每升高1℃,基础代谢率就升高13%。人在长期饥饿或营养不足时,会出现基础代谢降低。此外,测定基础代谢率和在不同活动强度下的能量代谢率也是合理制定营养标准,安排人们膳食的依据。
