肝浊音界
hepatic dullness
指叩诊所得的肝上界、肝下界。被检查者取仰卧位或坐位,检查者沿右锁骨中线、右腋中线和右肩胛线由肺区叩向腹部,当由清音转为浊音时,即为肝上界。正常肝上界的相对浊音界一般在右锁骨中线第5肋间水平,下界位于右季肋下缘;右腋中线上其上界为第7肋间,下界相当于第10肋骨水平;在右肩胛线上,其上界为第10肋间。肝下界为一薄层组织且与胃、结肠等重叠,很难叩准,故多用触诊确定,有时还需要配合叩听诊法或搔听诊法。肝浊音界向下移位见于慢性肺气肿、右侧张力性气胸等。肝浊音界向上移位见于右肺纤维化、右下肺不张、气腹和鼓肠等。
