风温 冬春两季感受风热病邪,初起以发热、微恶风寒、口微渴、咳嗽、脉浮数等症状为特征的外感热病。因春季阳气升发,温暖多风,最易形成风热病邪,若此时起居不慎、寒暖失调,使外邪侵入则发为风温。风温在发展过程中有顺传和逆传两种情况。顺传指肺卫之邪不解,内传气分。逆传指肺卫邪热,逆传心包。临床常见证型有:①邪袭肺卫型风温。证见发热微恶寒,咳嗽,舌尖红,苔薄白,脉浮数,治宜辛凉解表,方用银翘散或桑菊饮。②热在胸膈型风温。证见身热心烦,口渴尿黄,或便秘或胸脘痞满,苔黄,脉浮滑数,治宜清热凉膈,化痰开结,方用栀子豉汤、凉膈散或小陷胸汤。③邪热在肺型风温。证见身热烦渴,咳喘胸闷,苔黄脉数。治宜清热宣肺,方用麻杏石甘汤加味。④热在阳明型风温。证见壮热心烦,口渴,汗大出,苔黄燥,脉洪大,治宜清热生津,方用白虎汤。⑤逆传心包型风温。证见身热神昏,舌蹇肢厥,脉细数。治宜清心开窍,方用清宫汤。⑥内闭外脱型风温。证见身热昏愦,汗出气短,倦卧肢冷,脉细微。治宜清心开窍,固脱救逆,方用安宫牛黄丸合生脉散。 |
