适应症: 支气管哮喘以及伴有慢性支气管炎和肺气肿的可逆性支气管痉挛,对夜间发作的哮喘患者更适宜。
用量用法:
成人 一般剂量为200mg/日,晚上8-9点服用或早晚各服用100mg,根据病情可增加剂量至最大用量600mg/日,3岁以上儿童10mg/kg体重/日。
禁忌: 对茶碱不能耐受的病人,未治愈的癫痫患者。
不良反应:
头痛、恶心、失眠,少见消化不良、震颤和眩晕。
注意事项:
消化性溃疡,肝肾功能不全,心力衰竭,持续高烧及使用某些药物,有低氧血症,高血压病史的患者慎用。孕妇及哺乳妇女避免使用。茶碱的毒性常出现在血清浓度为15~20μg/ml,特别是在治疗开始,早期多见的有恶心、呕吐、易激动、失眠等,当血清浓度超过20μg/ml,可出现心动过速、心律失常,血清中茶碱超过40μg/ml,可发生发热、失水、惊厥等症状,严重的甚至呼吸、心跳停止致死。本品可通过胎盘屏障,也能分泌入乳汁,随乳汁排出,故孕妇、产妇及哺乳期妇女慎用。新生儿血浆清除率可降低,血清浓度增加,应慎用。老年人因血浆清除率降低,潜在毒性增加,55岁以上患者慎用。
药物相互作用:
与别嘌呤醇,西咪替丁,环丙沙星,红霉素及口服避孕药合用,可使茶碱血浓度增加。与利福平合用时,可使血清浓度降低。与苯妥因合用时,二者血药浓度均降低。勿与非选择性β-受体阻滞剂同时使用。
