此类患者蛋白尿与体位无关，即直立位及卧位时均排出逾量的尿蛋白，但也可于直立位时加重。一般无其他症状，血沉、血液化学、肾功能及肾组织学均属正常范围。在普查中此类可占无症状蛋白尿患者的5%～10%。临床上对持续性蛋白尿诊为“良性”应十分慎重，因某些肾小球疾病或其早期可仅表现为持续性蛋白尿(如系膜增生性肾炎、膜性肾病、肾硬化、糖尿病肾病等），常需加强随访及进一步检查。
