此癌组织形态上与其他器官的分化不良癌相似。巨细胞的直径可达200mμm，有丰富的嗜酸性胞浆，细胞核形态怪异，染色质粗，核仁突出，常见多核巨细胞。电镜及免疫组化染色证实为上皮性来源。瘤巨细胞呈松散的片状分布，并与梭形细胞形成肉瘤样结构及混合存在的腺癌成分互相移行。有时多核的瘤巨细胞很像滋养细胞，诊断时需与绒癌区别。后者一般发生在生育年龄，组织学上有典型的细胞滋养细胞和合体滋养细胞的双相分化。预后差，死亡率高。
