即非化脓性中耳炎。多见于小儿，特点为中耳内有渗出性、非化脓性积液，导致听力下降、有耳闷胀感、耳鸣等，鼓膜暗淡无光，混浊如毛玻璃油纸状，可有凹陷，可见液平面、气泡影等，治疗需针对病因处理。局部行鼓膜穿刺抽液，理疗、鼓膜切开术及乳突手术等疗法。
