髌腱断裂
rupture of patellar tendon
见于举重和体操运动员，系举重时或自单杠或高低杠落地时膝关节突然屈曲，股四头肌突然猛烈收缩所致。常伴有膝关节侧副韧带、交叉韧带或半月板撕裂伤。伤部多位于髌腱的近端止点附近。受伤时膝前有受击感和响声，不能主动伸膝。卧床膝关节屈曲90°位时可见髌骨上移、髌骨下方有一横行凹陷，局部及腱周压痛，可伴有膝关节积血。膝关节屈曲位X线侧位摄片可见髌骨上移、髌腱软组织影失去连贯性，晚期病例可见髌下脂肪垫钙化影。早期可用张力钢丝牵拉髌骨下降、钢丝固定于胫骨结节6周，同时直接缝合断端，并用股四头肌腱腱瓣或股薄股腱加固。晚期病例必须先恢复膝关节的活动范围、髌下脂肪垫钙化骨化影清晰之后再手术修补髌腱。
