Objective To summarize the nursing experience in caring patients with lumbar intervertebral disc herniation who received percutaneous lumbar discectomy(PLD) together with intradiscal electrothermal treatment(IDET) under DSA guidance.
目的总结DSA引导下经皮椎间盘切吸术(PLD)联合椎间盘电热疗法(IDET)治疗腰椎间盘突出症的护理经验。

经内镜注射硬化剂疗法
transendoscope injecting hardener
是经纤维内镜将硬化剂注入食管曲张静脉或其破裂处及其周围，使曲张静脉栓塞而达到控制出血的目的。适用于不宜手术的危重患者。近期止血率达90%。可在急性出血时或经三腔二囊管压迫止血及用升压素初步奏效6～24h内使用。硬化剂有5%乙醇胺油酸盐、2.5%～5%鱼肝油酸钠、3%或0.5%～1.5%十四烃基硫酸钠、乙氧硬化剂等。方法：先作内镜检查食管、胃底的出血灶，然后在破裂水平的曲张静脉内或其周围注射硬化剂4～5处，每处一般2ml，不超过3ml，尤其是静脉周围注射者。2～5min即止血。每周1次，4～5次为一个疗程。不良反应有低热、心动过速、胸骨后痛、食管溃疡或穿孔或周围漏或狭窄、胸腔积液或化脓等。再出血率为40%左右。
