主动脉瓣替换术
aortic valve replacement
先天性或后天性主动脉瓣病变，后者以风湿性最常见，造成主动脉瓣狭窄和/或关闭不全者，瓣膜损害严重而不能采用修复瓣膜手术时，应及时进行主动脉瓣替换术。手术在全麻体外循环下进行，取主动脉根部前壁斜横行切口，切除病变的主动脉瓣，瓣缘应留2mm。选用口径较大的人工瓣，如双叶机械瓣或生物瓣，应用带或不带垫片的2-0双头无损伤针线作外翻褥式缝合，在左、右冠状窦处的缝针应缝在冠状动脉开口下方，在左、右冠状窦交界处进针不应过深，以免损伤传导束。置入人工瓣膜后，检查瓣叶是否开关良好，有无堵塞冠状动脉开口。若瓣环小者，可将人工瓣膜放置于瓣环上方，或采用多种方法扩大瓣环后，再置入人工瓣膜。应用机械瓣膜者，术后应终生抗凝治疗；应用生物瓣者，术后应用3～6个月抗凝治疗。手术死亡率为2%～10%。
