因肾动脉狭窄或阻塞引起的高血压。是肾性高血压的一种。占高血压的5%~15%。血管病变可以是先天性,动脉粥样硬化性或炎症性。本病的高血压发展快,多无高血压家族史,一半患者上腹部或脊肋角处可听到血管收缩期杂音。非选择性腹主动脉造影和选择性肾动脉造影可确诊,进而作出手术或腔内成形术的治疗选择。术前进行分侧肾静脉肾素活性测定,对诊断本病和预测手术治疗效果有重要意义。对无法进行上述治疗,或上述治疗无效时可用降压药物治疗。
