二尖瓣狭窄直视分离术
open mitral commissurotomy
又称“直视二尖瓣交界切开术”。适用于左心房内有血栓、二尖瓣再度狭窄、高度怀疑有二尖瓣关闭不全可能施行瓣膜替换术,或伴有重度三尖瓣病变需同期纠治者。手术在气管内全麻体外循环下进行。方法是切开融合的瓣膜交界,分离瓣下腱索及乳头肌粘连,刮除瓣缘钙化灶及左房血栓。伴三尖瓣病变者可同期纠治。条件许可,应列为二尖瓣狭窄外科治疗的首选方法。手术死亡率在2%以下。
