〗应置于监护病房(ICU
即通常说的非手术治疗。外伤性脾破裂的非手术治疗，成功率在30%～85%，儿童成功率有报告可达100%。适应证：①年龄小于50岁；②非开放性钝挫伤；③仅有脾外伤，无其他腹内脏器挫伤；④血流动力学稳定，输血不多于2～4单元，检查指标稳定，无凝血障碍，B超观察血肿不再扩大；⑤临床症状逐渐好转。非手术治疗方法患者：应置于监护病房(ICU)中，绝对卧床休息，禁食至少24h以上，持续胃管及输液维持水和电解质平衡、输血(量不超过40ml/kg)，保持心血管功能稳定，使脉率不超过100次/min，收缩压维持于12kPa。非手术治疗者至少3个月内禁止剧烈运动或强烈劳动，以免再出血。非手术治疗期间，出现下列情况之一者，必须手术：①腹痛和压痛加重，甚至有腹肌强直等腹膜刺激征者；②48h内输血超过4个单元或24h内超过每千克体重40ml，血流动力学仍不稳定者；③在动态观察中血细胞比容稳定24h后又下降6%以上，或血细胞比容水平又下降至25%，输血2个单元不能迅速纠正者；④观察期间发现腹内其他严重的合并伤者。
