脊髓背外侧束切断术指切断组成背外侧束的Aδ纤维和C纤维，以阻断后根进入脊髓时其痛觉传导到脊髓后角，从而达到止痛目的。手术是在椎板切除术后，暴露脊髓，用小尖刀在脊髓背外侧沟的内侧1mm处刺入，向外侧切割2mm，深度也达2mm。
