产后急性子宫内膜炎
病原体经胎盘剥离面侵入,扩散到整个子宫蜕膜层,引起急性子宫内膜炎。炎症往往累及紧邻的表浅肌层,感染扩散时更易侵及深部肌层乃至浆膜层,因此,子宫内膜炎多伴有子宫肌炎。轻型患者炎症主要局限于子宫内膜层,局部充血、水肿、白细胞浸润,内膜坏死,有大量脓性分泌物。临床表现出低热,伴有下腹隐痛。体温多不超过38℃,脉搏稍快,恶露量多,混浊而有臭味,有时呈泡沫状。子宫复旧延缓,宫底有轻度压痛,质较软。炎症若能及时控制,坏死内膜组织将在数日内剥脱,内膜重新修复,症状消失。重症者,病原菌大量繁殖并迅速向宫旁组织、腹膜扩散,甚至出现菌血症。全身症状严重,突然出现寒战、高热、头痛、嗜睡、脉速、白细胞显著增高,子宫复旧延缓,压痛有轻有重,恶露不一定很多,臭味也不一致。由于缺乏典型的局部体征,容易造成误诊。治疗方面主要增加营养,采取半卧位,重症少量多次输血,高热时对症处理,主要选用敏感的抗生素。
