是指神经元过度放电从一开始就累及两侧大脑半球，脑电图一开始就有双侧异常放电，临床发作都有意识障碍，肌肉抽搐或肌张力丧失也呈两侧性。全身性发作有多种表现，如失神发作、不典型失神、肌阵挛性发作、阵挛性发作、强直性发作、强直-阵挛发作、失张力发作。
