腹膜间皮瘤
为腹膜原发性肿瘤,可分为局限型和弥漫型两种。前者多为良性,后者为恶性。可发生于任何年龄,但以40~60岁为多,男多于女。局限型肿瘤呈坚实灰黄色结节,基底连接壁层或脏层腹膜,结节大小不等,小者如钱币,大者占据大部分腹腔。弥漫性肿瘤广泛分布于壁层腹膜,并可侵及脏层。腹膜腔内常有浆液性、浆液血性或血性积液。肿瘤包围或压迫脏器,并填塞腹膜腔。镜检可见多量平行排列的胶原纤维束,间有细长梭形的瘤细胞,细胞核呈椭圆形,常有异常核分裂。肿瘤可侵犯局部组织,少有转移,有时可转移到附近淋巴结。临床多无表现,偶有腹痛,若腹痛剧烈,多提示弥漫型。局限型可手术治疗,弥漫型可采取化学药物治疗,但效果差。
