系以支架(托马斯架)上端的铁圈抵触于骨盆的坐骨结节，作为牵引时的反作用支撑力。另一端用骨骼或皮肤牵引与上端的固定呈拮抗作向下牵引患肢，可以缓解软组织的紧张和回缩，使骨折或脱位复位，预防和矫正畸形。
