目前称“感染性心内膜炎”。因细菌、真菌和其他微生物(如柯萨奇病毒、立克次体等)直接感染而产生的心内膜炎。以往认为其感染的病原为细菌,故称为细菌性心内膜炎。其有急性和亚急性之分。原无心脏病的患儿,遇有毒力较强的细菌。发生败血症并发心内膜炎,病程在6周以内者为急性;在原有心脏病的基础上感染毒力较弱的细菌,病程超过6周者为亚急性。其典型的临床表现有发热、心脏杂音、栓塞现象、皮肤病损、脾肿大、血培养阳性等。血培养是确诊的关键。治疗应争取及早应用大剂量抗生素,不可等待血培养结果而延缓治疗。但在治疗之前先作几次血培养和药物敏感试验的指导抗生素的选用。
