impulsive neurosis
冲动性神经症(impulsive nourosis)亦称“冲动控制障碍”(impulse eontroldisorders)，患者反复出现压倒一切的强烈冲动，去做一种对自己对他人不利的行动。事先感到愈来愈大无法抗拒的压力，迫使其采取行动。行动时感到愉快和满足，紧张解除，过后有时感到后悔和自责。常见的类型有盗窃癖、放火癖、拔毛癖和病理性赌博。盗窃癖(kleptomania)较多见于妇女，虽然精神健康而且生活富裕，仍无法克制反复去偷一些对她没有价值和用途的东西。放火癖(pyromania)多为男子，经常幻想失火和火灾后的情景，并反复纵火;但并不是为了获得保险金、销匿罪证、表示对社会不满或出于愤怒和报复，也不是受幻觉妄想所支配。拔毛癖(trichotilliomania)反复拔自己的毛发，使毛发显著稀少，并不是由于精神发育迟滞或幻觉妄想支配，也不存在皮肤炎症。病理性赌博(pathologieal gambling)者沉溺于赌博之中不能自拔，赌博愈来愈频，赌注愈下愈大，即使债台高筑，生活和工作受挫也在所不顾。放火癖为了逃避惩罚，多采取否认态度，成人放火癖有些被关入监狱，难以获得系统治疗，故有关治疗的报告甚少。对儿童放火癖应严格监护，防止对社会造成巨大损失。对其他冲动性神经症可采用行为疗法，如脱敏及厌恶疗法，同时配合支持性心理治疗及自知力治疗。转变环境，开展娱乐、文体活动以取代有害活动，也有重要作用。 (赵亚志撰刘协和审)
