5) Illioinguinal/Iliohypogastric

髂腹股沟淋巴结清除术
手术名。适于阴 茎癌伴腹股沟淋巴结转移(病理证实)，及肿瘤恶性程度较高或侵犯范围较广，腹股沟淋巴结 肿大高度可疑癌转移者。但若原发癌肿无法切除或有远处转移；盆腔或腹部有固定之转移灶 ； 下腹及下肢有淋巴阻塞或术中发现癌肿浸润髂股部血管、神经及骶神经者属手术禁忌。硬 膜外麻醉或腰麻，取髂前上棘内侧斜行切口，至腹股沟韧带中点弯向下方至股血管内侧，将 淋巴及脂肪组织自腹外斜肌腱膜表面锐性剥离。范围上缘由髂前上棘至腹股沟外环内上方 ， 自此两点垂直向下达股三角下缘。沿阔筋膜表面分离至股三角内缘，切断大隐静脉并剥离其 周之脂肪、淋巴。切开阔筋膜，在股三角下方切开股血管鞘，剥除股管淋巴结，从髂前上棘 内侧向内下切断腹股沟韧带，钝性推开腹膜达髂总血管分叉上方，切开髂血管鞘，将盆腔内 脂肪、筋膜及淋巴组织整块切除。缝合腹股沟韧带，移植缝匠肌覆盖股血管。同法处理对侧 。两切口间戳孔置管引流。术后切口加压包扎。引流管接负压吸引，3～4天后拔除。下肢抬 高2周。抗炎治疗。
