减张缝合
减张缝合即张力缝合。适应证:①年老体弱,消耗性疾病(如癌症),因组织愈合能力较差,关腹时可加做减张缝合;②术后可能腹压过高、剧烈咳嗽、腹水、妊娠、肠麻痹等患者;③腹壁切口裂开再次缝合。常用腹膜外减张缝合,即先关闭腹膜,用大弯三角缝针,坚实的10号尼龙线,自皮肤贯穿腹壁筋膜下,每隔3~4cm一针,针眼距切缘2~3cm,暂不打结,将筋膜、皮下脂肪及皮肤缝合后,张力线套上长约2cm的橡皮管,或夹上小纱球,依次收紧打结。张口线一般保留10~14天。
