diffuse pigmented villonodular synovitis
病因未明。可能系炎症物质造成的繁殖性反应，介于炎症与良性肿瘤之间的滑膜疾病，滑膜绒毛状增生并有色素沉着，其中间有葡萄状结节。多见于中年人。男性多于女性。常有外伤史。膝关节为好发部位，髋、踝等关节也常受累，也可发生于窝滑囊或手的屈肌腱鞘。疼痛轻微，肿胀明显，关节内有较多的柔韧肿块。关节穿刺可吸出红棕色或血浆样液体。滑液内细胞检查或关节镜下病理活检，可见滑膜细胞和组织细胞内有含铁血黄素包含物。X线片显示关节肿胀，部分可见骨质破坏。弥漫性滑膜炎侵犯软骨或骨，或影响关节功能者，可彻底切除滑膜，否则容易复发。手术后复发两次以上者，可行放射治疗。
