下丘脑内分泌综合征 hypothalamic endocrine syndrome 主要表现为内分泌功能失调或伴有其他下丘脑-垂体功能紊乱的疾病。由下丘脑-垂体肿瘤,先天性、肉芽肿性、病毒性、退行性及血管性损害,外伤、手术及放射治疗损伤,药物影响以及功能性等原因造成。病变部位可位于垂体、垂体柄、正中隆突或高级神经中枢。病变必须为双侧,方可导致下丘脑功能紊乱。 垂体水平病变可导致一种或数种垂体细胞、激素及其受体缺乏,产生相应的垂体功能低减,而负反馈地影响下丘脑内分泌功能。垂体柄水平病变可引起尿崩症,以及生长激素、促甲状腺激素、促肾上腺皮质激素及促性腺激素分泌减少和高催乳激素血症引起的相应症状。正中隆突水平病变可造成垂体激素分泌的昼夜节律消失或选择性垂体功能衰竭。高级神经中枢病变也可引起下丘脑-垂体功能障碍。此外,下丘脑内分泌综合征还可有一系列非内分泌功能紊乱的临床表现,如睡眠-醒觉周期紊乱、进食异常造成的肥胖或消瘦、渴觉异常、体温及排汗调节异常、记忆和行为状态异常以及植物神经功能紊乱等。 根据临床表现,选测脑脊液中下丘脑激素或血中垂体激素有助于诊断,多需进行下丘脑-垂体-靶腺动态功能试验,头颅CT扫描或磁共振对确定病变的位置和性质很有帮助。 应尽可能作病因治疗,包括手术治疗和放射治疗。可根据情况选用下丘脑功能调节剂如克罗米芬,下丘脑释放激素调节剂如溴隐亭、促性腺激素释放激素(LHRH)及其激动剂以及生长抑素等。有继发性垂体及靶腺功能低减时,需用激素补充替代疗法。 |
