补充资料:子宫腺肌症
      正常位置的子宫内膜向肌壁内良性侵入的一种妇科疾病,伴随子宫肌层的弥漫性增生。过去称为内在性子宫内膜异位症,但其病因、发病年龄、临床表现、处理原则均与外在性子宫内膜异位症不同,多发生在40~50岁经产妇。其原因是内膜基底层直接向肌层扩展,不是由于种植或化生。
  
  异位的子宫内膜弥散于子宫肌壁,后壁多见。肌纤维弥漫性反应性增生。子宫均匀增大,质硬,经期前后大小可有改变。常合并内膜异位症、肌瘤和盆腔炎性粘连。
  
  临床主要表现为继发痛经进行性加重,经期延长月经量多。诊断根据为典型症状、子宫均匀增大、子宫造影(如宫腔增大、边缘不齐呈毛刺状或不规则充盈缺损,造影剂进入肌层),应与子宫肌瘤鉴别。
  
  若患者年轻、症状不重,可观察和对症治疗,孕激素、雄激素、棉酚等可减轻症状。症状严重或无须保留生育功能者可作子宫切除。
