| 食管炎 esophagitis 食管发炎。其病因和种类很多，如反流性食管炎、念珠菌性食管炎、疱疹性食管炎及放射性食管炎等，其中最主要的是反流性食管炎。反流性食管炎系指胃及十二指肠内容物反流至食管引起食管下段粘膜的炎症。正常人食管下端存在一括约肌，即下食管括约肌，可阻止胃及十二指肠内容物的反流。当此括约肌功能障碍（压力降低或松弛）时，则可引起反流。反流的盐酸、胃蛋白酶、胆盐可损伤食管粘膜导致食管炎。临床上本病可单独存在，亦可以与食管裂孔疝并存。胸骨后烧灼感为最常见症状，多在夜间平卧时出现，坐起则缓解。有时可出现胸痛、出血及咽下困难。内镜检查可见食管粘膜充血、糜烂，甚至溃疡。慢性反复的损害可引起食管纤维化，造成狭窄。治疗包括抬高床头、戒烟吸烟能明显降低下食管括约肌压力）、应用降低胃内酸度的药物（抗酸药、组胺H受体阻滞药及质子汞抑制药等）及促胃动力药，狭窄者可做扩张术，无效时可考虑手术。 |
