保留迷走神经胃左、右的肝支和腹腔支，切断供应胃壁和腹腔食管段的所有迷走神经分支，从而避免了其他内脏功能紊乱的可能。但胃潴留的问题仍未解决。为此，需加行胃引流手术：①幽门成形术；②胃-空肠吻合术；③胃窦或半胃切除并行胃-十二指肠吻合或胃-空肠吻合。该手术加引流术治疗溃疡的效果较满意。但手术复杂，加行引流手术虽避免了胃潴留，但倾倒症状和腹泻的可能性仍存在。手术适应证、切口、麻醉和并发症见“胃迷走神经切除术”。
