主、肺动脉完全起自解剖学的右心室，或肺动脉骑跨于作为左心室惟一出口的室间隔缺损之上，二尖瓣前叶和主动脉根部被肌肉组织所分隔，多伴有肺动脉口狭窄。无狭窄者其表现与室间隔缺损伴大量左向右分流颇为相似，多数患儿早期出现动脉高压及右心室肥厚，亦可有左心室肥厚，P-R间期延长和室内传导阻滞等。超声心动图显示二尖瓣前叶与主动脉后壁不相连。心血管造影可确诊。
