脊髓灰质炎 poliomyelitis 由脊髓灰质炎病毒引起的急性传染病。因常引起小儿的弛缓性瘫痪,故又称小儿麻痹症。1954年美国科学家Q.索尔克发现造成脊髓灰质炎的病毒,研制出皮下注射疫苗,后A.萨宾研制成功可终生免疫的口服疫苗。该病毒属微小核糖核酸病毒科肠道病毒属。有Ⅰ、、3个血清型,3型之间有一定的交叉免疫反应,但不完全,因此应用疫苗预防时,必须3型都用;又由于3个型别的病毒在肠道中繁殖时可互相干扰,因此3个型别的疫苗应当分3次服用,每次间隔6~8周。病毒主要通过粪-口传播,侵入人体后主要在淋巴结及血中繁殖,仅1%~2%能侵入中枢神经系统,故绝大多数病人表现为顿挫型(类似感冒),病毒侵入神经系统后则可引起无菌性脑膜炎,也可引起瘫痪,少数病人可遗留瘫痪后遗症。口服减毒活疫苗的预防效果很好,如果小儿能普遍服用即可控制该病。疫苗为活疫苗,须冷冻保存。 |
