先天性髋关节脱位 congenital dislocation of hip joint 因髋关节发育不良,而于小儿开始练习走路时逐渐发生的髋关节的半脱位或全脱位。包括出生前及出生后的髋关节脱位以及可能脱位的体位不稳状态。根据病情进展,分为体位不稳、半脱位及全脱位3型。臼发育不良及脱位时出现非生理性剪力,半脱位时臼外缘出现应力集中,日久关节面磨损加重,导致早期出现骨关节病。此病在体位不稳及半脱位期症状不明显,单侧脱位时,两大腿内侧或臀褶不对称,患侧内收肌紧张,外展受限,脱位复位脆响征(奥尔托拉尼氏征)阳性,患肢 站立时健 侧 骨盆下降(特伦德伦堡氏征阳性)。X射线片可确诊。治疗原则是早期诊断早期治疗。应用外展架或蛙式石膏变非生理性剪力为生理性压力,有利于关节塑形。大龄儿童的臼发育不良及半脱位可行臼盖成形术,扩大头臼负荷面,变应力集中为应力均匀。 |
