皮质脊髓束
corticospinal tract

  是人脑中最大的下行纤维束。它起始于大脑皮质中央前回和皮质旁中央小叶的前部,经内囊、中脑和脑桥障入延髓锥体,在锥体下端大部分纤维交叉至对侧,在脊髓侧索内下行,直达骶髓。这部分交叉的纤维称皮质脊髓侧束,而没有交叉的小部分纤维,在同侧前索中下行,居前正中裂两侧,一般下行不超过胸节,这部分纤维称皮质脊髓前束。两束最后终止于脊髓前角运动细胞。此束主要控制骨骼肌的随意运动。当一侧的皮质脊髓束损伤时,病灶的同侧伤面以下肢体产生痉挛性瘫痪,肌张力增高,腱反射亢进,以致出现阵挛,病理反射阳性、浅反射消失等症状。
