肾体积比正常小50%～70%。特征是含有正常的肾皮髓质，肾单位和导管的分化和发育正常，但数目减少。单侧性先天性肾发育不良症，只在体查时触及对侧代偿性肥大的肾脏而作排泄性尿路造影时，或因并发高血压查因时才被诊断。小儿慢性肾功能不全时常因双侧肾发育不全引起，晚期严重高血压患者，只有肾移植治疗。近年B超产前诊断检查法的普及，发现并发羊水过少的胎儿，一侧或两侧肾发育不良的胎儿出生前诊断越来越多，出生后定期监测，早期出现血压增高的患儿单侧病变者，如健侧功能、解剖正常，确诊后可考虑行单侧发育不良的小肾切除术。
