左心室肥厚
指左心室肌纤维增粗增长,左室壁增厚而言。多由二尖瓣、主动脉瓣关闭不全,高血压等疾病引起。临床检查发现心尖搏动向左下移位,心界向左下扩大。心电图检查,其除极面积和电偶数目增加,除极向量增大,心电图各导联QRS波群电压较正常为高,心前导RV5>2.5mV,RV5+Sr1>3.5mV(女),4.0mV(男),肢体导联可有RⅠ>1.5mV,RaVL>1.3mV,RⅠ+S'Ⅲ>2.5mV等。复极程序与正常也不同,出现S-T向量和T向量异常,因此相应导联可有ST-T的改变。S-T下降0.05mV以上,T波低平,双向或倒置。但QRS电压增高,诊断左心室肥厚的特异性较差。一般将仅有QRS波群增高诊断左心室高电压,如同时伴有ST-T改变者,则可诊断为左心室肥厚,若仅有反映左心室导联的ST-T改变者,称"左心室劳损"。
