瘫痪多不对称，呈下神经元性(弛缓性）瘫痪。腱反射常消失，肌张力多减退、感觉多不受影响。瘫痪最常见于下肢，其次为上肢。近端大肌群常较远端小肌群瘫痪出现早且重。影响呼吸肌(膈肌及肋间肌）时则影响呼吸运动，表现为呼吸浅速、鼻翼扇动、声音低微、讲话断续、咳嗽无力、胸廓扩张受限(肋间肌瘫痪），吸气时上腹不外凸反内凹(膈肌瘫痪）等。重则发生严重缺氧现象。
