输卵管积水的发病机制尚不清楚,有人认为是慢性输卵管积脓的脓性渗出物被吸收后,残留的液体所形成,但大多数人则认为是由于毒性较低的细菌上行性感染所致。细菌主要感染输卵管黏膜,当伞部黏膜因炎症粘连闭锁后,聚积在管腔内的漏出液和渗出液逐渐增多而形成输卵管积水。输卵管积水多呈扭曲的腊肠或曲颈蒸馏瓶状,但一般与周围器官无粘连或仅有少量稀薄的粘连。因输卵管壶腹部管壁肌层薄弱而峡部肌层坚厚,液体多积聚在壶腹部,以致输卵管远端膨大而近端相对细小,故输卵管积水可以以近端为轴发生扭转。扭转后所产生的临床症状与卵巢囊肿蒂扭转的症状相似,术前往往难以鉴别。患者发生急腹痛时,往往行急诊手术切除患侧输卵管。
