| 旋毛虫病 trichinosis 旋毛线虫寄生于人体引起的一种人畜共患的寄生虫病。假旋毛线虫也可引起。成虫寄生于小肠上部的粘膜上皮层内，雌虫受精后产卵，虫卵在其子宫内孵化为幼虫，幼虫排出后即钻入淋巴管或小静脉并随体循环到达全身各处，主要在横纹肌，尤其是膈肌、舌肌及其他活动较多的肌肉内成囊，其囊壁由宿主的组织反应形成，囊内幼虫可存活多年，但成虫寿命仅1个月左右。鼠、猪、犬、野猪、熊等均可为贮存宿主，人们常因进食不熟的带虫猪肉或其他肉而受感染。临床征象分为3期，肠期为幼虫在肠粘膜内发育为成虫，此时可有恶心、呕吐、腹痛及腹泻；移行期的症状有发冷、发热，体温与感染程度往往平行，颜面浮肿，全身肌肉肿胀和疼痛，以腓肠肌为明显，眼肌活动受限，咀嚼、吞咽、言语及呼吸困难，末梢血液酸性粒细胞数明显增多，也可出现心肌炎、脑炎、脑膜炎等征象，此期死亡率较高；感染后第3周幼虫周围开始形成包囊称为成囊期，此时发热等症状逐渐减轻，并进入恢复期。诊断主要依据吃生肉史、临床特征及血清免疫学反应阳性，肌肉活检也有助于确诊；检测循环抗原有助于早期诊断及考核疗效。大剂量丙硫苯咪唑治疗有效。 |
