清初戏曲音乐家。字灵昭，苏州人，身世不详。精于音律。为洪昇所作《长生殿》传奇审音协律，守法甚严。洪昇《长生殿·例言》说："予自惟文采不逮临川，而恪守韵调，罔敢稍有踰越。盖姑苏徐灵昭氏，为今之周郎，尝论撰《九宫新谱》，予与之审音协律，无一字不慎也。"《稗畦集》有洪昇《赠徐灵昭》及《秋夜静德寺同徐灵昭》诗，内有"寺古山深秋气肃，空阶并坐夜忘眠"句，可见二人交谊之深。1688年《长生殿》定稿，徐麟为之作序。稗畦草堂刻本《长生殿》并载有徐的眉批127则,计：辨正查误32则，审音协律58则，用法标注33则,度曲提示4则。不仅对调误、字误和板误多所纠正,对字音、字韵、字句的协律,以及曲牌的结构、宫调、体格也有阐述。在度曲方面，分别指出何曲须细唱，不可用快板繁字;何句何字是务头,须作适当处理等。对后世作曲、制谱、度曲及研究《长生殿》的音乐，都有较高的参考价值。
