| 颈肩痛 neck and shoulder pain 由颈椎骨、关节、韧带、肌肉、筋膜及肩关节软组织病变或内脏疾病引起的综合征。又称颈臂痛。表现为局部疼痛。由于颈肩部解剖结构复杂，神经血管间关系密切，故引起颈肩痛的因素很多。颈部疾患中以退行性病变引起的为多见，其次为急性颈部软组织损伤、慢性软组织劳损、颈椎本身病变（如结核、炎症和肿瘤等）。肩部疾病引起的有肩周炎、肩袖断裂等。肩关节周围的骨折、肩关节脱位、结核及肿瘤引起的较少见。颈肩交界处有第一肋骨、锁骨和第一胸椎围成的间隙称为胸廓出口，臂丛神经和锁骨下动、静脉及前斜角肌等由此处通过。当胸廓出口出现解剖异常或肩下垂等因素，可能挤压神经血管束，称为胸出口综合征。神经系统疾病，如脊髓肿瘤、脊髓空洞症、脊髓侧索硬化症、进行性肌萎缩等病，以及内脏疾病也可引起，痛在右侧可能由胆石症、胆囊炎及肝病引起。痛在左侧的可能由冠心病或心肌梗死引起。根据病史、临床表现、神经系统检查、X射线检查来诊断和鉴别诊断。颈肩痛的治疗应针对病因进行。 |
