七氟醚麻醉
Sevoflurane anesthesia

为吸入七氟醚达到全身麻醉。七氟醚是一种新吸入麻醉药,其物理性质优于现有吸入麻药,血/气分数系数仅0.59,诱导迅速,且组织摄取少,苏醒也较快;嗅味好;但化学性质不很稳定,遇钠石灰可产生5种水解产物。与强效吸入麻醉药相比,七氟醚效能较弱,MAC值(1.7%)较高。其体内代谢主要经肝细胞色素P-450系统水解,颅内压已升高的患者不宜使用。对循环抑制也较轻,对心肌传导系统无影响,2%浓度可使心脏每搏指数、射血率、收缩率下降20%左右。七氟醚不刺激呼吸道,分泌物也无显著增加,但对呼吸的抑制强于氟烷。对垂体、肾上腺无明显影响,但可使胰岛素及胰高血糖素水平下降。临床应用和其他吸入全麻药相同,可用面罩诱导,维持浓度面罩患者为2.5%~4.5%,气管插管患者为1.5%~2.5%。
