| 足跟痛 calcanodynia 是由于足跟的骨质、关节、滑囊、筋膜等处病变引起的疾病。常见的为蹠筋膜炎，往往发生在久立或行走工作者，长期、慢性轻伤引起，表现为蹠筋膜纤维断裂及修复过程，在跟骨下方偏内侧的筋膜附丽处骨质增生及压痛 ，侧位X射线片显示跟骨骨刺。但是有骨刺不一定有足跟痛，蹠筋膜炎不一定有骨刺。用足跟垫有效。压痛点注射腊酸强的松龙，每周一次，往往2～3次治愈。久治无效的足跟痛可行跟骨砧孔减压术。跟垫痛常见于老年人，跟垫弹力下降，整个足跟下方都有压痛。海绵跟垫及封闭疗法有效。跟后滑囊炎常发生在跟腱与皮肤之间，摩擦损伤引起，表现囊内积液，肿胀压痛。避免摩擦及囊内注射醋酸强的松龙有效。跟骨骨骺骨软骨病发生于9岁左右的男孩，跟腱用力时疼痛及局部压痛。骨骺愈合后，症状自愈。距骨下关节炎常发生于跟骨骨折后，如保守治疗无效，应行跟距关节融合术。 |
