gastric ulcer in high position
指位于贲门附近的良性溃疡，多在小弯侧。约占胃溃疡的5%。手术有其特点：术前或术中须取活检排除贲门癌。如行溃疡切除加远侧胃切除可引起贲门狭窄；全胃或近侧胃部分切除，手术过大。可采用下述手术中的任何一种：①溃疡远侧胃部分切除、胃十二指肠吻合术(Kelling-Madlener手术)，如为大的溃疡不宜切除，胃部分切除后，溃疡可以自愈。②胃迷走神经切断、幽门成形术。
