子宫颈
解剖名。为子宫伸入阴道的一部分 ,包括子宫峡部和子宫颈内口以下的部分子宫,在子宫的前方。其上界几乎相当于腹膜开始 反折到膀胱上之处,被阴道的附着处分为阴道上和阴道两部分;阴道上的部分宫颈后侧被腹 膜所覆盖,而前面和左、右侧面与膀胱和阔韧带的结缔组织相接触,子宫颈的阴道部分伸入 阴道,其下端为子宫颈外口,外口形状因人而异:在未产妇,它是小而齐整的卵圆形孔;经 产妇因生产时受到一定的损伤(最易发生在外口两旁),使之变为一条横行的缝道,如此则将 子 宫颈外口分成前唇和后唇;如在初产妇宫颈遭到较严重的多处撕裂时,其外口变得很不规则 。根据这种撕裂的痕迹,可以无疑地诊断为经产妇。在禁止私自堕胎的国家,往往根据宫颈 上所遗残痕以辨认、鉴定非法堕胎残迹的真伪。宫颈钳痕,为扩张宫颈时钳夹所致;宫颈口 损伤,为强行插入扩张器所致;部分女性宫颈内可见胎儿残存组织。子宫颈主要由结缔组织 所组成,偶有平滑肌纤维;血管及弹性组织丰富。宫颈管黏膜为一层高柱状上皮所组成,它 处于一层薄的基底膜上,其黏液细胞为宫颈分泌的厚而黏的分泌物;排卵后,孕激素抑制 子 宫颈内膜分泌黏液,使之不利于精子或其他异物穿透。此外,由于宫颈管内膜腺体受雌、孕 激素的影响,使黏液拉丝度发生相应改变,可据此而选择性交时日。子宫颈在性反应周期中 ,随子宫体抬高而相应改变自己的位置,并于性高潮期略为松弛其外口。
