心肌梗塞
旧称"心肌梗死"。是由于冠状动脉闭塞,血流中断、导致心肌严重而持久的缺血、缺氧引起的心肌局部坏死。按临床过程和心电图的表现,本病可分为急性、亚急性和慢性三期,但临床症状主要出现在急性期中,部分病人还有先兆表现。突然发生或出现较以往加重而频繁的心绞痛,持续时间较以往长,可达数小时或数天,休息或含用硝酸甘油片不能缓解,同时伴有白细胞增高,血沉加快,血清酶增高,如肌酸磷酸激酶(CPK),谷-草转氨酶(GOT)、乳酸脱氢酶(LDH)等增高,严重病例可有休克,心律失常,心力衰竭,胃肠道反应等症状和体征。心电图可出现病理性Q波,S-T段抬高,T波倒置,且心电图有动态性衍变,具有确诊价值。心肌梗塞是急、危、重症,确诊后应积极抢救,应休息,给予镇痛、补充血容量、抗休克等对症治疗。
