位于安阳西北35公里处的清凉山村西侧，始建于北魏年间，因寺院内有一座形制独特的唐塔而闻名。整个寺院布局坐北朝南，有三重院落，主要殿堂有：天王殿、大佛殿、二佛殿及铁瓦殿，四座大殿排列得错落有致。
唐塔就在天王殿与铁瓦殿之间，是一座四方型是单檐亭阁砖塔，高约16米，外观朴素。塔身外壁刻满了各种雕饰，奇就奇在这些雕饰全部是用各种矩形、菱形、五边形、三角形等雕砖拼砌而成，内容既有佛教中的天王、力士、飞天等，又有道教中的真人、童子、侍女等，还有青龙白虎等民间传统图案。这种佛、道的大融合体现了唐代文化中外交融、兼收并畜的特点，是我国古代塔中的珍品。
