| 血液常规检查 blood routine examination 对血液细胞成分的检查。包括血红蛋白测定、红细胞计数、白细胞计数和白细胞分类计数，一般不包括血小板计数，有的医院也不包括红细胞计数。是住院病人的三大常规化验（血、尿、便）之一，对判断病人的状况及对疾病诊断和治疗都是有用的。 ①血红蛋白测定。血红蛋白是红细胞的主要成分，是判断有无贫血和红细胞增多的最重要指标。最常用的测定方法是比色法，过去及现在的一些基层医院多采用目视比色，此法简便易行，不需特殊仪器，但不太精确，易受主观因素影响，目前大多被光电比色法代替，其结果精确可靠。此外在工矿、农村等地也可用特定的试纸法测定，但准确性差。中国人血红蛋白的正常值为：男性成人120～160g／L（12～16g／dL） ，女性成人110～150g／L（11～15g／dL），新生儿为170～200g／L（17～20g／dL）。 ②红细胞计数。常用传统方法是显微镜下目视计数法，此法简便易行，结果准确，但目前有条件的医院多被血细胞自动计数器计数法代替，此法迅速简便，误差小。中国人的红细胞计数正常值 ：男性成人4～5×1012／L（400～500万／微升），女性成人3.5～5×1012／L（350～500万／微升），新生儿6～7×1012／L（600～700万／微升）。红细胞计数的临床意义与血红蛋白基本相同，但在巨幼细胞性贫血和小细胞低色素性贫血时，表示的贫血程度不如血红蛋白准确。 ③白细胞计数。与红细胞计数方法相同。正常值：成人4～10×109／L（4000～10000／微升），新生儿15～20×109／L（15000～20000／微升） 。除在生理情况下如妊娠、剧烈运动和酷热或严寒等能引起白细胞计数变化外，更重要的是许多病理情况如感染、组织损伤、某些血液病、肿瘤及化疗等均可引起白细胞计数和分类计数的改变，因而此项检查有非常重要的临床意义。 ④白细胞分类计数。白细胞又分为粒细胞、淋巴细胞和单核细胞，正常人各种白细胞间比例相对固定，而在各种病理情况下则可引起比例变化。经瑞氏和姬姆萨染色法染过的血液涂片，在显微镜下可清晰辨认各种白细胞，计数100个，算出百分率，即得出白细胞的分类计数结果。正常成人的血液白细胞分类计数值：嗜中性杆状核粒细胞1％～5％；嗜中性分叶核粒细胞50％～70％；嗜酸性粒细胞0.5％～5％；嗜碱性粒细胞0％～1％；淋巴细胞20％～40％；单核细胞3％～8％。 |
