各种原因所致脑细胞外液渗透压(包括血浆渗透压）降低，使细胞内含水量增加而发生脑水肿。此型脑水肿的水肿液就是水分。常见急性水中毒、低钠血症、糖尿病性酸中毒、抗利尿激素分泌增加等。水分主要聚集在神经胶质细胞内，白质及灰质均有水肿，以白质较明显，水肿区域内钠离子浓度略低，钾离子浓度明显降低。〔〕
