周围血中杆状核粒细胞增多并出现晚幼粒、中幼粒、早幼粒等细胞超过5%时,称为核左移,如仅见杆状核粒细胞>6%,称轻度左移;杆状核粒细胞>10%并伴有少数晚幼粒以致中幼粒,称中度左移;杆状核粒细胞>25%并出现早、中、晚甚至原粒细胞者则为重度左移,严重左移又称“类白血病反应”。核左移常见于急性化脓性感染,急性中毒、急性溶血时亦可见到核左移现象。
